The Friends & Family Test will be on-going and you are welcome to complete the survey following any contact, for any reason, with the practice. You can find the Friends & Family Test on the side panel to the left of this page to complete our form.
We would like you to have the best possible experience of care from us and the new NHS Friends & Family Test is a way of gathering information to enable us to review the care and service we provide. The survey is based on one simple question plus an optional follow up question, and the practice has added a third question in order to gain as much feedback as possible regarding what you like and what you think we could improve.
Alternatively, paper copies of the survey are now available at reception and in the waiting rooms at both sites, together with a collection box for your completed forms.
