Practice Nurse

Job Description

We are looking for an ambitious Practice Nurse to join our team at East Harling and Kenninghall Medical Practice, based in South Norfolk, caring for approximately 8,800 patients.

We are a growing nursing team with a wide range of disciplines and an ambition to be the leading team in the wider area. Whatever your level of experience, we are willing to train the best candidates in the various specialisms. We are looking for an enthusiastic and motivated individual with excellent communication skills who is able to work autonomously and as part of a team. The ability to travel across the 2 surgery sites is necessary.

Essential skills:

  • Registered Nurse, current NMC registration

Desirable knowledge and skills:

  • Knowledge of QoF
  • Qualification/Experience in Long term condition management – asthma/COPD/diabetes
  • Able to direct and co-ordinate programmes of care working autonomously and collaboratively, recognising when referrals are required
  • Organised, friendly, approachable, self-motivated
  • Adaptable and comfortable dealing with changing priorities and a team player
  • Work within level of competence
  • Able to work in a changing environment and implement change when applicable to provide high-quality health outcomes

Clinical Responsibilities:

  • Assess, plan, provide, implement, and evaluate nursing care to meet the care needs of individuals in the practice population.
  • Identify and manage as appropriate, treatment plans for patients both at risk of developing a long-term condition and diagnosed chronic disease.
  • Support patients to adopt health promotion strategies that encourage patients to live healthily and apply principles of self-care.
  • Undertake health screening, surveillance and immunisation.
  • Assess and care for patient with presenting wounds including leg ulcer management
  • Vaccination and immunisations, administering injections, ensuring safe vaccine storage, implementing cold chain policy, working with PSD and PGD guidance
  • Women’s health
  • Venepuncture
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including the initiation of effective emergency care
  • Adhere to practice policies and procedures
  • Agree own actions are consistent with agrees standards of care, following practice / national guidelines
  • Assess own performance and take accountability for own actions either directly or under supervision

Job Type:

Part-time (Negotiable)

Salary:

£17.00 – £23.00 per hour

East Harling and Kenninghall Practice is approximately 26 miles south of the City of Norwich and cares for approximately 8,800 patients. It operates from two separate surgeries – East Harling: a small market town and Kenninghall: a rural but vibrant village. We are a growing nursing team with a wide range of disciplines and an ambition to be the leading team in the wider area. Whatever your level of experience, we are willing to train the best candidate.

  • Adhere to practice policies and procedures
  • Agree own actions are consistent with agrees standards of care, following practice/national guidelines
  • Assess own performance either directly or under supervision
