Community Team

There may be occasions when patients require the services provided by the district nursing team. The scope of their work is to assess and deliver nursing care, and to support patients and carers in their own homes who are unable to attend the surgery because of medical reasons. Health Visitor

The health visiting team (Health visitor, nursery nurse and support worker) are available to provide support, advice and information for all in relation to health issues, but in particular to families with infants and young children.
